The eye is a complicated
organ. Eye health is an
important part of
achieving overall health.
Your eye exam and a wide selection of
frames and contacts are fully covered.
Optical benefits include:
• A thorough and professional eye exam*
• A wide selection of frames fully covered
up to retail value of $175** (If patient
chooses a frame above $175, a credit of
$80 will be given)
• Prescription plastic lenses include single
vision. Bifocal, trifocal and standard
progressive (includes cosmetic tint, ultra
violet coating, scratch resistant coating
and oversize)
OR
• One pair of daily wear (single vision,
non-cosmetic, non-astigmatism) from
Amsoft Hard or Soft, Bausch & Lomb or
equivalent (Fitting and follow-up visits
included, fitting is not covered for non-
covered contacts)
